How can I obtain my pathology slide(s) and report(s)?

Typically, your provider will contact our laboratory directly and provide us with the necessary signed authorization forms including a specific address to send material. You may also choose to pick them up in-person at our office if you make the necessary arrangements with our staff and sign a release form.

Why do I have to sign a release form?

An authorization form must be signed to comply with HIPAA regulations which ensure the security and privacy of your protected health information. To learn more about HIPPA click here  HIPPA.

How long will it take for my request to be processed?

For recent cases, your request will be processed and fulfilled within 5 business days from the day it is received.  For older cases, processing and fulfillment might take slightly longer.

Will you mail my pathology records or will I have to pick them up?

We will Federal Express your pathology records to authorized facilities, unless other arrangements have been made with our staff.

What are your hours for release of pathology records?

After arrangements have been made with our staff, records can be picked up between the hours of 9:00 a.m. and 5:00 p.m. Monday – Friday.

How can I get more information regarding my pathology report?

How do I request a copy of my pathology report?

Please contact your provider for a copy of your report.

Will you fax a copy of my pathology report to my home?

No, we will not fax any medical records to patients’ homes.

Why is Medical Laboratory Services billing me?

We will bill for our pathology diagnostic services separately from your physician.  If there is a question regarding your bill, please feel free to contact our exceptional on-site billing specialists for assistance. M-Th 8:00 a.m. – 3:00 p.m.
